The R.A.I.N. Case Management Program serves homebound older adults 60 years of age and over residing in Bronx Community Districts 7,8, and 12. Case managers assess, coordinate, monitor, evaluate, and advocate for multiple services and interventions to meet the needs of the older adult.
Our case managers are trained professionals who assess and link older adults to a wide range of services such as transportation, housekeeping, home care, home delivered meals and assistance with entitlements and benefits. The program strives to provide excellent services with the goal of helping seniors to continue to live safely in their homes and to re-engage in their communities. We strive towards the integration of care delivery.
